Chronic Disease Prevention

Provides information, education and classes to help individuals and groups understand, prevent and/or manage a variety of chronic health conditions.

Our Community Health Outreach team provides trained professionals to assist educating individuals, groups, organizations and communities in a variety of chronic conditions.

Community Health Equity and Inclusion

Community Health Equity and Inclusion addresses health equity issues concerning county residents with the greater goal of decreasing health disparities and inequities in Tarrant County.

Community Youth Development (CYD)

The CYD program was established for the purpose of reducing juvenile crime in areas of Texas with the highest incidence of youth crime.

Maternal and Child Health Programs

Maternal Child Health Programs provide services that support the health and safety of women and children and helps with connection to other resources in the community. This includes the Safe Riders Program, Prematurity Risk Prevention, the Child Fatality Review Team and others.

Nurse-Family Partnership Program

This program partners young mothers with experienced nurses to improve health outcomes for their children.

Women, Infants and Children (WIC) Nutrition Program

WIC provides nutrition assessments, healthy foods, nutrition, breastfeeding education and referrals for eligible pregnant, post-partum and breastfeeding women, as well as infants and children under the age of 5.
