    TCPH DSRIP Projects

    Antenatal Steroid:

    The purpose of this project is preventing/reducing preterm birth, through the use of antenatal steroids, in a population of women that have a history of spontaneous preterm birth.

    Chronic Disease Self-Management Program (CDSMP):

    The purpose of this project is to increase the health-related quality of life (HRQOL) of hypertensive and diabetic residents of Tarrant County by implementing the evidence-based Stanford Chronic Disease Self-Management Program (CDSMP).

    Falls Prevention:

    The purpose of this project is to improve the Health Related Quality of Life (HRQoL) through the implementation of the evidence-based, Matter of Balance Fall Management Program. The program is designed to teach clients the necessary fall-management techniques that will help reduce and manage falls.

    Health Information Exchange (HIE):

    The purpose of this project is to improve population health through more effective use of e-health information, including the use of syndromic surveillance related to chronic and communicable diseases, use of Electronic Lab Reports (ELR) for notifiable/reportable disease monitoring, and the use of immunization data for preventive outreach. Specifically, the goal of the project is to more effectively prevent the spread of Pertussis in Tarrant County.

    PRIDE (Arlington):

    The purpose of this evidence-based intervention is to inform and educate youth about sexually transmitted diseases (STD’s) and how to make appropriate choices regarding sexual activities, thereby reducing the level of disease (STD’s) in the community.

    STD Clinic Expansion (Arlington):

    The purpose of this project is to reduce the incidence of sexually transmitted diseases in the City of Arlington through increased service hours, thereby increasing availability of clinical and epidemiological services.

    TB Medication/Rifapentine:

    The purpose of this project is to reduce active TB in the community by modifying the delivery regimen and medication to those with latent TB infection (LTBI). This will be achieved by combining the current isoniazid (INH) treatment with the newer drug Rifapentine (RPT).

    Tobacco Cessation:

    The purpose of this project is to use the American Lung Association’s (ALA) evidence-based Freedom-From-Smoking (FFS) curriculum to decrease tobacco use in Tarrant County.

    Tuberculosis (TB) Clinic Expansion:

    The purpose of this project is to provide non-traditional service hours to improve access to care and compliance of treatment of latent TB infection, thus reducing active TB in the population.
