You must download the Reporting Form, complete it, and fax it to 817-850-2305 or email it to SJsteen@tarrantcounty.com
Note that any proof of attendance is acceptable. This form is for your convenience.
You must include your full name and County Court at Law No. 3 cause number to ensure you are properly reporting.
You report must include the date, time and alcohol counseling group you attended. Each meeting you attend must be verified by the meeting leader or Chairperson.
Occupational Driver's License Driving Log
If you are required to maintain a log you may use a spiral notebook, hardbound notebook or print out the sample log below. You MUST keep all pages together and you MUST number each page.
