To obtain a copy of a Birth Certificate the following information is required:

• Full name as appears on birth record
• Date and Place of birth
• Parents name including mother’s maiden name
• Your relationship to person named on certificate
• Your mailing address
• Purpose of record
• In person you will need to provide your original valid government issued    I. D.

In order to request a birth certificate you must be one of the following:

• Self
• Parent
• Step Parent
• Grandparent
• Sibling

If not one of the above, the requester must have:

• A signed notarized original statement authorizing and naming them to pick up birth certificates.

• Statement must include a copy of your government issued identification with the person named in your request.

We accept checks, cash or credit cards.

Long-form birth certificates are available at the Tarrant County Plaza location only.

Sub-courthouses can provide Abstract (short-version) only.

The hours of operation are Monday - Friday 8:00 a.m. - 4:30 p.m.
